What to bring to your first appointment:

  • Your referral letter from your GP or referring doctor

  • Any past test results including blood tests and scans

  • A list of your usual medications

  • A completed Patient Registration Form and Patient Health Survey (unless completed online)

  • Your Medicare card

  • Private health insurance details if applicable
